Before they reveal the note, the viewer says plainly: the sender will be notified when this note is opened. No silent tracking, ever.
When they reveal it, your iPhone gets a generic push — "A note was opened." — and the Sent tab records the opened-at time. Up to five of your devices can register for a note's notifications.
